KN95 Distribution Pre-Registration

The Governor recently announced that his office, the Delaware Department of Education, and the Delaware Emergency Management Agency secured over 125,000 masks for students in grades 6-12, staff hired and employed by schools, and licensed childcare providers across the state. DEMA and other partners will start the distribution of masks on the days listed below. Please note that this is a one time distribution of one KN95 mask per full time employee and students in grades 6-12.

This form is for licensed childcare providers, charter schools, and private schools to preregister to pick up KN95 masks. Only one person from any given licensed provider or school should complete this form. Please do not complete this form if you are not an authorized representative for a provider or charter or private school. Public schools should not complete this form because distribution will occur through your school district.

Your facility is responsible to pick up the masks from the distribution location below on the day and time listed. In order to pick up the masks, school representatives will need to bring a school ID and childcare providers will need a copy of their state childcare license.

We cannot deliver or mail masks directly to you.
